Running Patient Reports

  1. Select the Patient Report green heading, right click and choose New.
    The Report Criteria screen displays.

  2. Select the Report type by checking a box.
    • Encounter - The report includes registration details, address (all communication numbers), and gives a one line summary of each clinical record including the most recent record(s) in each category. It includes problems, disease registers, medical history, recalls, referrals, consultations, acute and repeat issues, prevention data and absence of condition.
    • Home Visit - Displays registration details, address, Problems, Repeat Masters and Consultations.
    • Health Check Card This contains registration details, address, problems, allergies, repeat masters, recalls, BP, smears, weight, height, contraception, immunisations blood group and absence of condition.
    • Medical History - The last ten Medical History records display, the header details the patient's name, date of birth, sex, NHS / CHI number and registration status.
    • All Clinical – This includes problems, BP, referrals, smears, smoking, alcohol, weight, height, contraception, notepad entries, and Family History. The header details the patient's name, date of birth, sex, NHS / CHI number and registration status.
    • Therapy History - The last ten Acute and Repeat Issue records, and the header includes patient name, date of birth, sex, NHS / CHI number and registration status.
    • Full - This includes the main registration details all their clinical data and includes the patient's name, date of birth, sex, NHS / CHI number and registration status at the top of every page..

    Note - The Encounter, Home Visit and Health Check Card Reports prints the last MAIN address recorded for this patient in Registration, to ensure that there is no confusion for home visits. All recorded Communication Numbers are also included.

  3. Tick to Print All Headers, this shows headings for each category, including where the patient has no record. This can act as a prompt that the patient has no relevant entry.
  4. Select either Single Patient or Group.
    • Single Patient (default), select the Select Patient button to choose a patient, and select OK.
    • To select a group, tick Group, the view changes to the Group Select view.
      Select the Select button, and the lists of Patient Groups and Clinical Audit Searches display in separate tabs.
      Select the group or clinical audit line, and select OK.
      The view refreshes and the selected group displays at the bottom of the screen, together with the number of patients in the group.
  5. Enter the From Date, for example, T-2m for the last 2 months, or leave blank if you do not want to apply a from date.
  6. Type the Search Name in the box, to save the report for future use, for example Medical History in the last 12 months.

    Note – You cannot apply a From Date when a Search Name is selected. If the box is left unticked, the report can be saved with the date criteria but the new report name does not display in the output.

  7. Select Save to update the search with the new criteria.
    Or use Save As to save the report as a different name.
  8. Select Print, the following options display:

    • Window - The default option, displays the report on screen.
      From here you can print, navigate to the next or previous page, select Two Page format, Zoom In or Zoom Out.

      Select Close to exit.

    • Print Options to select a printer
    • File - select from the drop down menu to save as a .TXT file.
  9. Note - When printing a report for a patient who has more than one page of freetext in their journal, the comments section is truncated to fit on a single page and the following text displays: "Some data has been truncated to fit page."

View previously run Patient Reports

  1. Select the green Patient Reports heading to expand the list of saved reports.

  2. Select the Patient button next to the report name.
    The report displays.
  3. Amend the Patient Report Selection Criteria if required, or select Print to run.
